Who should complete this form

Use this form for setting up direct deposit of your U.S. Treasury check into your MECU account. You could have received this U.S. Treasury check for any one of the following reasons:

  • Federal salary/mil. Civilian pay
  • Social security
  • Supplemental security income
  • Railroad retirement
  • Civil Service Retirement (OPM)
  • VA compensation or pension
  • Military active
  • Military retired
  • Military Survivor


Directions

  1. Type in the information according to the instructions on the "Direct Deposit Sign Up Form" in PDF Format* (* PDF is a file format for Adobe Acrobat Reader®. If you do not have Acrobat Reader, please download from Adobe®.)
  2. You must complete section 2 by typing in the government agency name and address
  3. Print the form
  4. Sign it and mail to the following address:

Note: Sending this form directly to the credit union allows the credit union to expedite its processing. After processing the form, MECU will then send the form to the appropriate payroll division.
