IMPORTANT INFORMATION ABOUT PROCEDURES FOR OPENING A NEW ACCOUNT

To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account. What this means for you: When you open an account, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver's license or other identifying documents.

Instructions to complete the Account Card for opening an UTMA account.

Step 1. Verify that the child is eligible for MECU membership. In many cases a child is eligible because the minor is a relative of an existing member. See the membership section of the web for a full description of eligibility requirements.

Step 2. Print out the attached “Account card” for opening an UTMA account.

Step 3. Print this page of instructions to help you complete the account card.

Step 4. Then complete the account card as follows:

  • In the Member Name section ...write in the name of the child in whose name the account will be opened. Include the full address, date of birth and child’s social security number. Also in this section include the reason why the minor is eligible for membership.

  • In the Name 2 section ...write in the name of the adult who will be the custodian of the account after the word “custodian”.

  • If you wish to designate a successor custodian… write that name in the UTMA Successor Custodian line. The successor custodian’s relationship to the minor should be noted in the Relationship to Member line.

  • In the Additional Account Services box, the custodian can also select access for the account via Telephone Teller, an ATM card and/or PC Banking.

  • Sign and date the form on the “Signature/Name#2” line and mail it with your initial deposit of at least $5 made payable to Municipal Employees Credit Union of Baltimore to:
