The P.O.D. Designation/Change Form enables you to designate the person(s) who will receive your account funds in the event of your death. You may also update this information at any time using this form.

To complete the P.O.D. Designation/Change Form, just follow the 3 steps below:

Step 2: Print the form and complete it.
