Directions

Step 1: Print the form and complete the information into the top section of the form:

  • Account Number
  • Name
  • Address
  • Old Rate
  • New APY
  • New Rate

Step 2: If your CD is for 48 months or 60 months, indicate in the second part of the form whether this is your first or second upgrade.

Step 3: Be sure to sign the form, and bring the form to any MECU branch during normal business hours and speak with a Member Services Representative, or mail the form to:
